doubt about filing the forms

I and my DH is applying I485, MY DS is a citizen of USA. Should i mention his name and details while filling the form I-134, I 485, I-131, and I 765. Please ILs your have gone through the situation, Kindly needed your advice. :bowdown

And i have one more doubt. My First name is as per passport starts M******* and Surname is G******* R******* ( my father's name which has space between the two( i.e. Gopala Krishna)). Which applying I 485 and the rest of the form, How should i mention my Last name either G******* R******* OR G******* OR R*******. This is big confusions for me. Im stuck in that. IF i mention my last name as R******, will my application get rejected, Pleaseeeeeeeeeeeeeeeeeeeeeeee:drowning
