Ration Card is a must

Hi All,

Please let me kow the process for renew my ration card.we got our new ratio card after marriage with only our 2 names(me and my DH). but that got cancelled because we have not got any items when i went for delivery before 2 years.Now we need to include my daughter's name in the card and renew it with new address.
1.what are the documents i need for it ?
2.what is the formalities for the same to renew.?
3.where is the office for the madipakkam zone.
