Please help me understand/select USA insurance plans.

Hubby has relocated to Pittsburgh coz of job.

I will be joining him 8months later. Have baby of 4 montsh(Will be 1yr old once i go usa)


MEDICAL

CGI will match employee HSA contribution up to a max.
$500 per course of the year divided into 24 payrolls.
• Two medical plan options
• HSA Option available
• Prescription Drug coverage
• Preventative Care covered Employee Contributions per month are as follows:

Opt 1 - HSA / Opt 2 - OA POS
Employee Only $141.35 $264.10
Employee + Spouse $281.00 $557.01
Family $409.99 $774.73



My doubts are:

1. Will above mean it includes hospitalization charges.
2. Will it include consultation charges
3. Will it include Medicine charges


** I understand dental,vision and few things not mentioned in Insurance is not covered.
