Do i need spouse endorsement for getting passport for my daughter?

hi,

i wud like to know the procedure for getting passport for my 50 days old daughter. Me and my husband did not have the spouse name in our passport, is that necessary for getting passport or can we proceed without spouse endorsement.............Experienced ladies plz guide thro' this........
