    Hi,

I have severe lower jaw pain and swelling for more than 4 weeks. It started off after I did a lot of chewing one day and next day jaw and ear pain started. Along with swelling. I went to an urgent care and got antibiotics for sinus and ear Infection. After 2 weeks of antibiotics, my sinus infection cleared up. My ear pain, lower jaw pain with swelling still present. It was paining in the lower jaw under the ear and in the upper jaw. Went to an ENT to check it out. He checked my ear and sinus and there is no infection.

Then I waited for few more days. Went to dentist last Friday to see if it's any dental issue.

They took an x-ray and no Infection present. Dentist said even though any infection was there, it would have been cleared with the antibiotics i have taken. I suspect it could be because of upper wisdom tooth which I haven't removed yet or TMJ issue. I removed my lower wisdom tooth a decade back.

I hear snapping or clicking sound when I open my mouth Everytime and not able to open wider. It hurts to open wider. Dentist said it's not TMJ and it will not give swelling. I don't know if I can trust him..he is a new dentist and I can't trust him though. He checked bite using blue stick and I felt discomfort and tenderness with one tooth. So he said that tooth in the lower side(#30) might be the reason and he is guessing it have some crack or fracture in it.

He told me to take cbct scan for that tooth to get more images. And he is not sure after the scan too. He said he could see some little thing and he guess it is a crack or fracture. He want me to extract that tooth since RCT will not be successful all the time. It's better to extract than waiting and watching after RCT.

I was totally shocked and worried to hear that. I don't want to jump into extraction with a guess and not a concrete answer. I do have upper wisdom tooth which has to be removed. X-ray show some black color stuff in that tooth and he told me to remove that too. That's fine. But the lower molar where I don't feel any tooth pain, I don't want to do RCT or extraction. I am planning to get a second opinion.

Did anyone have any experience of wisdom tooth removal of RCT here in the USA?

I dont know if I can wait and see if it resolve on it's own. I have the same level of swelling for past 4 weeks and it's still the same. I do have throbbing pain in the upper jaw(complete upper molar areas) and lower jaw under the ear.
    Thanks.

this seems to be your problem . If you cannot trust a doctor seek second opinion . You have a complicated health history . Internet is the last place you should seek advice from. Doctors are there for reason .

Get a second opinion from an endodontist. They are the ones who should be doing the root canal anyway since that is their speciality. I have had several root canals done here and a family member is now having whole mouth reconstruction done so I have seen dentists more often than I care to recently.

    Thank you so much @MalStrom.

I will try to consult an Endodontist. I don't know anyone who did RCT here. So I have to rely on Google reviews for the Endodontist.
Have you had crack or fracture in the tooth before which is not visible in the x-ray and for naked eye?
Did you ever extract your wisdom tooth here? Is it safe here or should I do it in India?

I currently have a tooth that my dentist suspects is fractured, based on X-rays. She sent the images to a specialist who wants to do a CT scan. The tooth will most likely need to be extracted and an implant placed, so I am dragging my feet on the next steps.
I got one wisdom tooth extracted here and it was a very easy process. If your teeth are impacted it can be more of an ordeal. Dental treatment is quite expensive here even with insurance, but I was happier with the quality of care compared to India.

I removed all my 4 wisdom teeth in the US and I did a few root canal treatments. Most of the time they take a full mouth x-ray before the treatment plan. Root canal with crown is very expensive, even with dental insurance.

    Thank you.

I am having the same issue. My dentist suspect a crack or fracture in a tooth. But it's not visible in xrays. He said cracks will not be visible on xrays. How did your dentist saw the crack in the x-ray?
I had CT scan for that tooth to get more images and my dentist said he couldn't see anything from the CT scan. He still see some tiny thing and he is guessing that might be a crack or fracture. That was first molar. Tooth#30.

I also have upper wisdom tooth infected on the same side. Tooth#1. So I have to remove that too. I am just scared to do it during this pandemic time. I don't have any family or friends here for the support. So little worried about it.

I have done 2 root canals and one wisdom tooth removal here in the US. I was two root canals were done by two different dentists and let me tell you. second one was an endodontist and it was much smoother than the first one. I had to do a second root canal on a near by tooth just because my other dentist nicked the adjacent tooth while performing the first root canal. In two months I had the similar symtoms as you are describing and I called my new denstist on a public holiday and she prescribed antibiotics and got the second root canal done with an endodontist later. Didnt hurt a bit. Cost was almost the same for both.
My dentist tell me all the time to take my other 3 wisdom tooth out, but I am not going to do that until it poses me any problem. It doesnt hurt nor is it sensitive, so I will not take them out until it is a problem and that is me. IMO, dentists always find something to extract or treat everytime I visit them. Is is just me? I dont know. So I go with what i want.
If I were you, and this tooth doesnt pose me a problem, and if I can still chew on it as normal, my answer would be no, i will not be doing root canal on this.
This is true. Root canal is not always successful. Your dentist will have to tell you for sure if it is a crack or fracture present, speculation is not good.
This is worrisome. See another dentist for a diagnose if you can. Please don't do any treatment unless they give full information on what is going on with you. Otherwise, it is too much waste of money.

Dentist and specialist suspect a fracture because I had a gum infection near that tooth. I have already had two root canals on that same tooth so there is no pain. My dentist told me I could extract the tooth and just leave it as is, but it’s near the front so the gap will show. If you’re not concerned by the cosmetic aspect you can ask about the extraction. FWIW I had my root canals done 22, 15 and 10 years ago. The 22-year old one is the only one currently bothering me. Go to an endodontist, not a regular dentist if you decide to go this route.
