Adding spouse name in Indian Passport

Adding Spouse Name in my passport

Hi,

i want to add my spouse name in my passport for that

1. what are documents required?
2. whether they will issue a new passport or they will include in old passport?
3. i don't have an bank account, so what other documents i need ?
3. Can i provide LIC document instead of bank a/c ?

Please help!!!!
