    Hi all,
I have jaw pain under the ear since may. Been to many doctors, dentists and no diagnosis yet.
My dentist suspect that that's is a tooth root fracture and want me to extract lower first molar. My symptoms are on the lower jaw under ear and neck , ear pain, ringing in ear. He said, I can try extracting and see if it helps. And implant after that. He said it could be TMJD or tooth fracture issue.

I had a second opinion with Endodontist and he said no to extraction. He couldn't spot any tooth fracture and told me to do root canal as that tooth have sensitivity to bite and cold.

I have the same symptoms till now. I went to Endo and had root canal in September. Still the same symptoms. Went back to my dentist ten days back and had core buildup, temporary crown.
Since then, I have very sore pain in that gum and tooth. It's like sore, sharp pinch like pain scratching like pain on that tooth and gum. Is this normal to have sore pain on that tooth after ten days of temporary crown? I am going to get permanent crown next week. I don't know if I should wait to get permanent crown to this pain settles down.

I also saw a tmj specialist and he said my pain and symptoms are from tmjd, muscle tightness, myofacial pain. He want me to try his treatment for mouth guard which is 4.5k$ and not under insurance. I am still thinking and deciding upon that. Meanwhile, have to see if this tooth is also a culprit for my pain.

My dentist still believe that my symptoms might be from tooth fracture and he want me to extract it if this crown doesn't help. He said implant has to be done immediately and it's around 6k$.
Anyone had crown before? Does it pain after temporary and permanent crown for a long time? Is this normal?
I did ask my dentist office and they said to wait till next appointment and see if it subsides.

I have had 4 root canals and crowns. I never had any pain once the root canal was done and the cavity was filled. I developed a gum boil on one of the root canal teeth after 20 years. The dentist initially told me it might be infection due to a tooth fracture. She suggested extraction and implant. I did not want since implant is $5k out of pocket. The infection seems to have reduced after antibiotics.
In your case I would find out the root cause of pain before adding permanent crown.

    Thank you @MalStrom.

I have to try tmj treatment mouth guard. But the specialist want me to finish all dental work before his treatment. Because the crown might change the bite impression. So, I didn't go for tmj treatment yet.
I have a neurologist appointment too to check if it's neuralgia to rule that out.
Don't know if this permanent crown will help. I have the appointment for permanent crown next week.
I would have tried tmj treatment before this root canal. I missed to do that. I was focusing more on this tooth issue and did rct to see if it helps. I don't have cavity at all on that tooth. Suspecting a tooth root fracture. But Endodontist who did rct said, he couldn't see any fracture and want me to go with crown and not extraction. But the dentist still suspect there might be a fracture in root. Difficult to get diagnosis and been delaying things. Constant pain since may. Don't know how to proceed further.

Based on our symptoms, I think it is TMJ. TMJ causes all these tooth issues, neck pain, headache, ear pain. Clenching and grinding teeth during the night cause TMJ. If you have a lot of pain when you wake up, it's definitely TMJ. You can try a night guard (available in Walmart or dentists can make one for you) and see. It's usually cheaper around 300-500$, not 4.5K.

    @RRAJ ,
I don't know which country are you in.
In the usa, the cost of tmj treatment is around 5 to 6k$. I have been to 3 different tmj specialist and they quote 5k, 5.5k and 4.5k$.
None of them will be in insurance network.

The dentist mouth guard comes around 700$. I asked my dentist last month and got a quotation. He told me that his guard is also not covered under insurance. And he recommended me to get one guard from the tmj specialist and not from him. He said , tmj specialist guard will be fine tuned with the joint. It's not very cheap as you think.

Thank you for the reply @drdiva . Appreciate it .
It was lower first molar. Tooth # 30(46 in India).
Does it indicates any fracture in that tooth? I have consulted a neurologist today to check if my pain is from any neuralgia.
Been to many doctors, dentist and ENT. No proper solution yet.
I do have sore ear pain, constant burning pain in lower jaw under ear and neck. Is it because of that lower molar or tmjd ?

That prove for the mouth guard made by the dentist sounds right. You can see whether you can put money in an FSA or HSA and use that for paying for the treatment. It will reduce your taxable income.

I have done 3 root canal till now.
No it doesn't hurt .there will be some discomfort soon after RCT for 2 days, that's all.
The crown has to be fitted on the root canal treated tooth asap. Usually dentists ask to do the crown fitting after 2 weeks from treatment.
The RCT tooth will be fragile, and you are not supposed to be chewing with it till crown is fitted
