cerclage failed - need your opinion

    hi divya,
    just want to say my best wishes to you....

    it is a nightmare when i think that even cerclage can fail.... i felt that women can walk fast and be active after the stitch but alas even then bed rest is required is news to me...

    and also i have the habit of sleeping on back, which i have to discontinue...

    hello vidya24 or anybody who knows the answer. Do doctors in India place mcdonald stitch or shirodkar stitch for cerclage procedure. I learnt from internet that shirodkar stich is stronger than Mcdonald Stitch.

    I did read about Abdominal stitch. The risk is that miscarriages in the earlier months also need C-Section. For example if no heart beat is detected around 3 months still you will have to go through c-section.

    Looks like you too read Wikipedia, like I did. I too asked my doctor the same question. They said these procedural issues are decided by the case.And things have progressed beyond all this. All we need know is that the stitch should hold.

    Also, miscarriage after encerclage need not be caused only by failure of the stitch. A wide, wide range of reasons including progesterone deficit can cause the pregnancy to fail, leading the foetus to push, causing the stitch to break.

    Any pregnancy -natural or assisted, early or delayed, high risk or normal- whatever be the nature, no pregnancy is guaranteed to safe delivery. This we all know. It is only after 32 weeks that I started relaxing. And after 37 weeks, I started relaxing more. And my gyne always told me that this was the case with any woman who came to the clinic.So, as one who has been in your place, what I would say is collect all the info you can, talk to your gyne repeatedly, face any surgical procedure during gestation with calm- then place your faith in the universe.

    Please don't think that I am advising out of place (though I sound like that). I have been in your place, much longer than you will be. Just leave it to the gyne and God, all will be well.

    Hi All,

    I also lost 3 kids, in 13 weeks, 18 weeks and the last one in 27weeks(10 days back). i am advised by my doctor to maintain a gap of one year to plan my next pregnancy since i underwent C-section this time. Can anyone help me to find a doctor who is expert in doing TAC in bangalore? I had two stitches in the last pregnancy, Mcdonald stitch in 12th week and Worm Stitch(thats what doctor said) in the 19th week but both are helpless, even after i survived with complete bed rest in hospital. I am completely depressed and not willing to talk to anyone even to my mom and husband but both are very very caring and supportive.

    Hi Priya,

    Which doctor did you go to for your pregnancies? If it is incompetent cervix why did you have a c-section. What is the reason for the miscarriage.
    Why did your cerclage fail. For how long were you in bedrest. Did you raise your legs while you were resting. Did your doctor give you progestorone shots after placing the cerclage.

    Since both of us have similar case and I am also planning to come to Bangalore for my next pregnancy it would be helpful for us to exchange some ideas. Let us make our next pregnancy successful.

    Sorry for the delayed response, missed the thread somehow. Yes u are right about TAC that miscarriages also need C-Section but IMO that's a small price to pay for holding a baby in the arms. But then its just my opinion and then my information is limited as well :). TAC can also be done during pregnancy, maybe after 3 months but u may want to ask an expert on all this. R'ber I gave u 2 email ids, why don't u just mail all the questions u have to those doctors. I sent my questions and both replied to them patiently.     I am so so sorry about the loss of ur baby. I understand what u are going through and ur not wanting to interact with people. i went through the same thing but then soon enough I realised that just thinking about the loss and cutting urself off from people will just aggravate ur pain. Talk. If not to ur mom, maybe to a friend who has been supportive (I have three friends who rallied around me during my hospital stay etc/ loss and were there for me emotionally anytime. One friend would also look up the net and I cud share my concerns even more than I cud with mom or even DH). But even if that's not an option pour ur heart out here. We are there to listen. Just don't bottle up. Grieve, accept and look forward to ur baby's soul coming back to u again. I am sure it will.
    About TAC, I am sorry I just cud not find any doctor in India who specialises in it. But in case u still are thinking of TVC do look for a perinatologist who specialises in high rish pregnancies.
    About waiting for an year, it's ur call but r'ber Ur c-sec is just like a normal c-sec and wud take normal time to heal. Just bec u lost the baby doesnt mean u need longer to heal physically (tho emotionally maybe u will). Usually the body takes about 6 months. But this is just my opinion. U and ur doc know best.
    Also, why did u need a c-sec? Did they not try to resuscitate the baby actively after delivery?
    Anyways best of luck to u. Hope u heal fast emotionally and physically both.

    Hi Friends,

    Thanks so much for your soothing reply, got tears while reading. Thanks friends. Yes i had C-secion just to save my baby. Fluid started coming out and since there were two stitches, doctor didnt want to take risk on Baby's life. Surgery was done immediately after 30mins after i informed the doctor about the leak. me and my husband were not sure whether normal delivery would be possible. just followed doctor's advice. i heard my child was crying for few seconds after delivery before he got shifted to incubator. He was taken to Manipal Hospital immediately and was there in NICU for 3 days, but due to premature lungs he could not survive.

    I was in bedrest since the first day of my pregnancy, but was in complete bedrest in the last two monts. Yes, i was taking Progestrone capsules vaginally twice a day but last one month was taking injection since capsule caused infection. The reason for delivery is CI. Ultrasound was done weekly and cervix length came down gradually everyweek as if there were no stitches. Last two scans showed abt ballooning of membranes and it made us worried so much. Doctor also lost hope and gave a steroid shot for baby's Lungs growth. But all in vain, my baby suffered 3 days in this world and happily flew off finally. The hospital bed rest was horrible. the Leg side of the cot was increased height of 1 foot and everything was in bed only, restricted completely to walk. i was consulting Mrs.Punitha Rangaraj in Cambridge hospital which in Cambridge Road.
    Divya, i came to know abt an TAC expert in Bangalore. He is a Gynec, Mr. Ramesh from Ramesh Hospital, Rajaji Nagar. Pls let me know if you know any other experts in bangalore.
    waiting4miracle, Thanks for your kind advice. Yes, its true, I need some time to recover from emotional thoughts. I am not getting proper sleep in night just becuase i lost all 3 babies in the night time only. I am physically week as well due to prolonged bedrest. My only hope is God is Great and is with me.

    I don't understand why your cervical length reduced inspite of putting the stitch and taking bed rest. Even I had weekely scans, my cervical length never reduced after that, it was always around 35mm. Do you think your doctor did not do a good job with putting the stitch. I personally feel that after the cerclage is placed vaginal progestrones are not good. Mine everything happened within a matter of 3 days, untill then everything was perfect. My cervix was completely open when I had miscarriage. Is this the first pregnancy for which you had a stitch. What about for your 2nd one. I see that from 13 weeks you could make it to 18 weeks for 2nd one.

    I am not sure if I really want to go for TAC. I still need to discuss with my doctor. Just the idea of c-section twice scares me (one for placing the stitch and one while delivery). Doesn't TAC get infected. What about bedrest with TAC. I am really confused. Did anybody suggest you TAC or did you get this information from internet. I am seeing the doctor next week, I will let you what he suggests me for next pregnancy. I will get to know what exactly is the reason for my cerclage failure only when he discusses all my reports. After that I will try to find best doctor for such cases. This site is really helpful. From this site I learnt that Dr.Shreedhar Venkatesh from Mallya clinic is really good. He also has his own clinic in RT Nagar. Just take an appointment with him to find out if he suggests and also to find out if does the TAC.

    Please do keep me posted what the doctors you are visting are suggesting you. I will also update you with my progress with doctors suggestions and any treatment. Just be strong Priya. I am completely concentrating on how to make it work next time.

    Waiting4miracle, have you decided to go for TAC. What are your plans with next pregnancy. Where are you planning to get it done.

    I was told by my doctor that my cervix is very week that is the reasone for the failure inspite of stitches. Yes, cerclage was done only in the last pregnancy. Last pregnancy was perfectly planned. I was in the US for an year,2009-10 and consulted a Gynec he took all the tests and said there is no problem in my body except CI. Once i reached india, i consulted Dr.Punitha as few colleages suggested this doctor. I started taking Folic Acid tablets 3 months before my pregnancy,conception happened in august and Doctor confirmed it on sep-1. I took 1 year leave and was in bedrest at home. At 11th week of my pregnancy, i felt litle pain and immediately rushed to the hospital. Doctor just examined vaginally and said cerclage is required as the Cervix is little open. I stayed in hospital for 15 days as per doctor's advice and the cervix length was 30mm during discharge. Doctor said it was normal and asked to continue the bedrest. At home, i walked only to use toilet and food was in bed in sitting position. Next consultation was after 15 days and the scans showed the lenght of 25mm. i didnot notice this scan report carefully and the doctor said everything was normal and scheduled the next visit after a month. i was scared and asked her to schedule after 15 days but she smiled and said i was normal and one month interval was ok. but after 15 days, i had little pain and went to the hospital. Dr.Punitha was not available and her assistant examined vaginally and said the stitch was perfect and asked me to come after 3 days to consult Dr.Punitha. But i requested her to take scan and the scan report revealed the cervix length of 12mm. I lost my hope completely there itself. Another assitant of Dr.Punitha examined me again vagianally and said the cervix was open and the stitch was loose. Second stitch was put on the same day which was on my 18th week. Since then i survived hospital bed rest and the cervix length was 19mm max after the second stitch.
    The hospital stay was not at all good as each doctor adviced in different way. One said i can stand up and took bath and the other said i can use toilet only to empty my bowel. Dr.Punitha didnot take it as seriously and each patient visit was a matter of 10 seconds and used to ask me "are you ok or how are you" and then she will say in Tamil "Ipdiye Irunthuko" which means "continue like this". The cervix length went down week by week and the leg side of the Cot height increased gradually after each scan. Still i am not sure why the length went down, thinking that my cervix is week in that extent.

    I was not at all motivated by the doctors and one day my mom went to the doctor without my knowledge and told her to use some positive words. Little change in Doctor's attitude after that. but the worst thing was that I was advised to use the toilet though my cervix length was 15mm. Doctor saw the scan report three days later and told me after that to restrict all my activities in the bed. Second worst was, nursing. It is like a government hospital and our call will not be answered immedialy, though the monthly expense of hospital stay was huge, around 70K per month approx. including medicines excluding operation charge. The only good thing happened was my s-section, they took my baby out very fast and sent to Manipal for further NICU treatment. I appreciate the treatment given in Manipal, it was really good though expensive. I was in much hope abt child's health on the next day of my delivery as he survived better but third day he started struggling for breathing. Manipal NICU care was better though the treatment was expensive, 1.25L for 3 days.

    The only prayer this time is, I have to get a good doctor who should do my stitches properly and also should motivate me mentally and directed me in the correct path to successfully survive pregnancy till 9months.
