Pre Admission Test Needs More Blood Due To Antibodies

    Seems that pre admission test needs more blood because
    1. My Dad’s blood has antibodies
    2. They don’t know what are these antibodies
    3. Need to figure out where to get compatible blood in case Dad needs transfusion during TAVR

    now there is confusion as to where my Dad has to go to get this done and it makes it so much complicated and tense!

    And I am scared what if they cannot get compatible blood type to whatever these antibodies are

    Does anyone have experience with this “type and screen” testing before surgery?

    The hospital should be able to tell you when and where to go for for the type and screen test. Mine was done at the hospital itself. Once they have the results they will be able to identify suitable donor blood for him.

    My Dad got it done in Hospital while he was inpatient for Angiogram

    More blood testing cannot be done inpatient, ie day of TAVR because it will be too late.

    it needs to be done 3 days before Wednesday, ie outpatient.Since outpatient center is closed on Sunday, it must be done on Monday

    But the problem is the Nurse Practitioner ( who is a sweetheart by the way) for the TAVR is unable to tell us accurately.

    She says he has to go to outpatient center. When I ask for something concrete in writing (such as appointment slip) she says,

    “don’t worry your Dad is in operating Room schedule, he is in the system”

    when I call the outpatient place they say he is not in the system.

    Then the outpatient place calls back and says my Dad has to go to hospital, not the outpatient area




    one hand does no know what the other hand is doing.
    This is LIFE AND DEATH,


    THe testing needs to be done within 3 days of the scheduled TAVR and because one hand doesn’t know what the other is doing, I am very scared and worried

    1. This life saving surgery will be delayed, increasing my Dads risk of death, stroke

    2. The Highly Qualified and Humble Doctor who my Dad wished to perform the surgery ( who himself suffered a loss in his family but was brave enough to come into the Hospital and do Angiogram), will not be able to do TAVR later on because of his religious holidays

    I called the Nurse Practitioner and she says she will fix this. Hoping to speak to her soon.

    i dont have much idea about this testing as for my daughter they did everything while she was inpatient there.but since dad was in the system for the procedure, they should be able to get all the information in the system..when you call and talk to person make sure you get their name which makes them sense that we going to call back and talk to them again, and eventually they will try more to help us...

    hope everything gets fixed soon and make sure you get an appointment for blood work so you dont have to wait for long time..

    Oh yes


    Now I called the outpatient center
    1. They have my Dad’s info
    2. He is scheduled for blood test at outpatient center (not Hospital itself,)


    Still have to verify few odds and ends because of this initial difficulty

    SD, good that the blood test thing is sorted out.
    That is often the case with hospital staff and anything related to the medical process such as insurance, billing, patient check-out from the hospital, pre-surgery tests etc. Sometimes, there is a communication gap even between the staff who was on duty in the previous shift and the new shift.

    It is natural to lose patience and get over-worried or worked up at the system and the person you are dealing with. We have to keep reminding ourselves:
    - to keep calm,
    - to focus on the immediate issue, and only that issue,
    - to understand the nurse etc are doing their best to help us
    - not to take out our frustration on the person we are talking to
    - be polite, persistent, and patient.
    - save one's mental energy for bigger decisions and concerns
    - remember to take deep breaths (no exaggeration)
    - accept that the system is inefficient and we have to work with what it is

    Uncertainty and lack of information are big drains on the mind. But these are part of things when we are dealing with medical stuff. Accept that there will always be some new challenge to fix and figure out, and you will conquer it, just as you have conquered so many in the past. There will be clueless staff who will give you the run-around and then suddenly there will be that compassionate nurse who will seek you out late in the night and ask how you are doing, if you have any questions.

    This is called catastrophizing. Look it up, learn to identify when you find yourself doing it, and gently shake those thoughts and send them on their way.

    TL;DR - Deep breaths, focus on the immediate, and be nice to the staff.

    Be nice to the staff - The doctor does the actual surgery but it is the staff who help you most after that. One time I had settled down in my chair next to my post-surgery patient, and suddenly two nurses came and started wheeling my patient away. They told me to gather the belongings bag etc. I immediately panicked. Gathered everything, remember to unplug my wall-charger etc and ran after the troupe pushing the stretcher and IV pole. It is very unnerving to be wheeled through public corridors, into elevators with other people. A nurse with whom I had made small-talk in the morning about her hours, job, family, quietly told me that they have to do this shuttling so that all rooms are used. Or it will look like the hospital does not need so many rooms. So, after four hours, they move a patient to a building 5 mins away by walk. Whatever. But knowing the reason helped me breathe easier.

    When I tell my friends in India about these things, and how we are made to run around, they find it hard to believe. One time, I waited exactly three hours with all bags packed, for the doctor to come and just say patient is ok to leave hospital. Later I came to know the delay is for making more money. Sigh.

    I totally agree about being g nice to hospital staff. They are under the pressure to take care of so many patients with little compensation.

    However I’m not catatostrophizing because Dr said better get this procedure done sooner rather than later. Valve is very tight and is is getting tighter and smaller. Dad was diagnosed last year and patients with his prognosis have 2 to 3 years to live if untreated.

    It’s one thing if nature made delays, we simply cannot control. But when humans (including computer systems made by humans) create delays, then it is more frustrating because it can be avoided

    Just now I straightened up more odds and ends.

    you are right when you say person from current duty may not know the challenges of person from previous duty


    It’s sad that some doctors care more about making money than the well being of their patients. We also came across this

    I’m still traumatized about how much pain my Dad has at catheter insertion site (wrist)

    wrist is black and blue and swollen

    But if it was leg the recovery would have been worse!

    I hope the surgeons can make an incision in his other wrist next week, else I cannot imagine how my Dad will tolerate laying down for more than 4 hours, unable to use restroom (he also has this type of health issue)

    You can discuss with the doctor or nurse. If he has difficulty with using the restroom perhaps they can suggest options like temporary use of adult diapers.
