Please Tell Me The Possibilities With This Semen Analysis

    Hello

    Need some suggestions from all of you please.

    One of my cousins ttc 15 months recently had her husband's semen analysis done. It showed 0 progressive motility(46% non progressive motility) and 1 morphology on the Kruger scale( strict morphology above 4).

    The Doctor says IVF with ICSI is the only choice in the scenario.

    Gave 20% chance of a successful pregnancy and delivery with this semen analysis per cycle rather than the normal 40% for ICSI per cycle. Said possibilities would increase with PGD up to 28% per cycle.

    Please advise.

    Cousin is extremely worried.

    Motility and count are very important factor for fertility.Morphology of 1% is still okay and can be ignored. However they still cause delay.
    When the motility says 0 it literally means 0 out of 100 sperms are moving correctly on their way towards the egg. (means in this case no sperm is actually making it's way in the right manner). The doctor is true in saying IVF with ICSI is the only option here.

    However try one or more of the following options :
    1) Take a second opinion from an other doctor and redo the sperm analysis. Different labs show different values. So no harm in taking second opinion.
    2) The doctor should be able to advice on supplements which will help in sperm motility. These micro nutrients boost the motility to an extent.

    Good luck to your cousin and from my experience worrying only worsens the situation and doesn't help at all in any way. So ask her to have a word with their RE and look for best options which would work for them .

    Count is 65 million which is okay. It's the only parameter which is normal.
    Her husband had a repeat semen analysis with the same result.
    Cousin's husband was a heavy smoker and has cut down on cigarettes recently.

    He had a spinal cord injury in his late teens which he worries might have altered his semen profile.

    He needs to sleep in a hard bed and had been using heat pads regularly which he has now reduced.

    If the semen analysis remains unchanged are chances of a healthy baby really bleak?

    They are willing to do ICSI or PGD as well

    Motility and Morphology are just the external factors which determine the quality of the sperms externally. Other things depend on the genetic information the sperm carries. I am not sure if motility actually affects this genetic information.
    Their doctor is taking them in the right direction by advising ICSI and PGD. ICSI helps in the fertilization of the egg and sperm.( In this case 100% required because sperm is unable to progress on it's own)
    Next is PGD - they test the embryo for any chromosomal abnormality and check if the pregnancy will be a healthy one . Whether motility affects this I am not sure. You need to have a word with the RE to check if motility affects the genetic information carried by the sperms or do your own research.
    Can't really answer that question with a "yes" or "no". Specialized doctors can guide them further.

    How does one check the quality of the genetic information?

    Not sure about sperm DNA fragmentation test.
    Pre implantation genetic screening/diagnosis (PGS/PGD) which you have mentioned earlier is the test recommended by the doctors to check the chromosomal abnormalities.

    Welcome. :) Lot of information available in net these days . But if your cousin is a kind who would get overwhelmed by so much information don't recommend her to check in the internet. Because in the course of understanding one thing we will end up reading something else and that'll put unwanted stress on us and we'll end up worrying a lot . Sharing my experience :) Sometimes just leaving it to the doctors is better.
