My Close friend is first time pregnant..her cervical length was 3 cm during NT scan at 13 weeks ..
And during Anamoly scan at 20 weeks she is found to have placenta Previa, I.e placenta lying 2 cms from cervix..cervical length was 2.4 cm..

Doctors didn't tell her to be on bedrest or prescribe any progesterone supplement or cervical stitch. Maybe becos this is first pregnancy..They gave standard precaution like no travel, no intercourse, no hectic activities etc..
She doesn't that've that much awareness about pregnancy related issues but she followed their instructions with just light household chore and light walk of 5-10 mins in common area of apartment after food..mostly sitting in bed reading books or eating or sleeping..
She has had vaginal pains, lower abdomen pains, cramping etc from the beginning..
She started having more pelvic pain and pressure after second trimester..
When she mentioned her concern about short cervical length and cramping they said that cramping and pelvic pains are normal, and that 2.4 cm is good no need to worry etc..
During 30 weeks completion they asked her to do scan,I.e Doppler studies..during that scan she asked radiologist to check cervical length ..he said baby is in cephalic position but unable to find cervical length..
Hen gynaecologist saw report they brushed aside her concerns and said that she s worrying too much ..
During 30th week she had considerable pelvic pains and was rushing to the loo 3 times at night and several times during daytime to pee.
After 30th week completion she is having some vaginal irritation, itching and watery discharge and got herself admitted..
The senior gynaecologist is not available, some junior gyne examined and scanned and told that cervix, internal os are open, maybe due to infection and cervix has ulcers..
They have admitted her and are giving her some medications through IV..continuously monitored by gyne, paediatricians, etc..she is in state of tension now..
1.when cervical length shortened from 3 CMs to 2.4 cm from 13 th to 20th week, why they dint recommend stitch or bed rest or progesterone suppositories..is the doctor right or she is negligent
    2. Is pelvic pain , cramping and pelvic pressure common in pregnancy...especially late pregnancy..Is pelvic pressure a sign of urinary infection maybe?
    3. Isn't transvaginal scan supposed to be done in her case to check the funnelling of cervix..
They should have monitored cervix in 30 weeks but they dint..
    4. Could this pre term labour symptoms have been avoided by proper monitoring?
    5. Was strep B test supposed to be done by that time..would a vaginal swab have given indication of infection..if it had been treated on time maybe this could have been avoided?
    6. Is cervical stitch routinely done for first pregnancy if c.l is less than 2.5 cm?
    7. Can short cervix cause membranes to leak?
She trusted her gyne when she said all was ok and now blaming herself for not taking second opinion right after 2 nd trimester scan..
The gyne is skilled and highly recommended..
Her folks are now looking for a second opinion..whether to trust this doctor or to go to different specialist for further treatment.
Can anyone answer these questions?Any inputs would be useful for them to converse with a specialist if they take second opinion..

My doctor did cervical stitch just to be on safer side(IVF pregnancy) and yes vaginal suppositories..

Better to take 2nd opinion... But I doubt if they will do a stitch now....Atleast now on let her be fully bed rest, get up for just bathroom n quick shower...

My baby was born at 33weeks...Since she has completed 30weeks it's good milestone.. Try to stretch 34 to 36 weeks with rest and doctor's monitoring all the best.

I am sorry to hear this, I pray for her speedy recovery and safe delivery.

@Needtobestrong, all your questions need to be addressed with a good obstetrician with medical knowledge to get second opinion. People here in forum can share their personal experience but every patient and scenario and treatment protocol will be different based on individual needs, so it is better to get professional help for all her concerns regarding the care she received from this doctor.

    Thanks @sumalynux and @daksh
It's so frustrating when a patient has concerns about health and doctors just brush it aside and don't give proper explanation..my friend feels that if proper attention had been given to her case she could have avoided this..she has been changing 3-4 pads a day and doctors din't even tell her properly if the fluid leaking is amniotic fluid or just watery discharge. Being in hospital bedrest it's difficult for her to even go to any other doctors clinic for second opinion.

    Hi

I do not have much experience in your other queries but i already have a low lying placenta of 2cm and even i was not advised bed rest. Though i still continue to rest.

My cousin had a similar experience like your friend. She was not advised bed rest and she was continuing to work. At 28 weeks she experienced contractions and was admitted in the hospital. The docs were planning for a cesarean. Her parents took her reports to another doctor whom they know. Looking at the reports she told that the baby can be delivered vaginally and so they shifted my cousin in an ambulance and this doc did normal delivery. The second opinion they got helped my cousin.

Though this isnt the response you are looking for. Am just telling that its not necessary she goes to another doc's clinic. Her family can take her reports and consult another doc to just double check the treatment that was administered to her.
