Urgent advise - SIL is unable to BF 3 days old twin sons

    Hello Friends,

    My S-I-L has delivered twin sons 3 days back. Mother and kids both are doing great. But the problem is my S-I-L is unable to BF them. Initially there was no milk for first two days (even a single drop). Now she is getting some milk though not enough for two kids but kids are refusing to suck her nipple. Her nipples are very hard. Can you please advise us how she can make her kids BF as they are only 3 days old and Dr is not letting her to give any thing else. Or any other alternative approach?

    I thought of Electric Breast Pump but seems these are very costly in India (I really had a very tough time with manual one..mine was just a local one bought from medical store), so can you please advise any reliable and easy to use Manual Breast Pump which I can order online ( as of now she in home town , a small city). I can also send e-pump from USA but it will take time and she is afraid thet milk may dried.

    Friends, please advise. She is feeling very guilt and depressed because of this issue.

    Force open the baby''s mouth and insert the nipple, this is what my nurse did!!

    Once they start sucking automatically the milk increases within few days,
    Tell her to make the baby suck both the sides n ask her to eat all tthe things yhe dietician adviced her esp. Garlic.

    And more over initially much milk doesn,t come. It increases as the child sucks.

    In olden days, they used to take the heat of fire and i remember my aunt doing so.
    I guess taking steam can also be done.

    Just ask your doctor abt it :)
    If these things doesnt work, u need to buy the electric pump forsure :) but dont b too late to take a decision.

    Since she has twins, definitely investing in electric breast pump is worth. Please dont think about it as waste. I also had the same issue while BFing initial days(rough nipple and semi flat). So my Dr suggested nipple shield for feeding. I used it for 8 days, then baby was able to latch with ease normally, as the issues got corrected by nipple shield usage. After that I stopped using it. Milk supply will increase once babies start sucking.

    that is tough. it is essential for babies to feed at the breast. that is the signal for the body to produce more milk. meanwhile, a pump will be necessary to get things going initially, foods with fenugreek, herbs like shatavari are very helpful to augment milk supply. mainly, babies need to be at the breast at this early stage. here is a website with some tips and articles that might help: http://kellymom.com/bf/got-milk/supply-worries/low-supply/

    dont worry...i also faced same problem. my child unable to suck milk for 15 days. but daily i used to try.
    tell her to sit calmly and push entire brown potion of breast inside the baby's mouth for sometime...thats is the only option...she will succeed...

    Initially, babies wouldn't know to latch properly. But ask her to keep trying. As babies suck more, the milk supply increases. When she brings the breast closer to baby's mouth, the baby will open its mouth and immediately put the nipple inside. I also had trouble initially and I used to sit for hours together for feeding. Once my DD learnt to latch, it's very easy to feed her.

    I didn't use a pump, so no idea about it.

    Hi,
    First of all, do not panic. I didnt have a drop of milk till the 5th day. First 3 days, when I was in hospital, doctors gave my daughter some multivitamin drops (looked like glucose water).Since I didnt have milk even after getting discharged, I gave her Lactogen on 3rd day night and 4th day. 5th day morning I had some milk secretion but I struggled giving her milk. She resisted and yes it was a tough time. The whole day I forced her over my breast and then in the late evening she got the art of sucking.
    So dont worry, ur SIL will have milk supply soon. I am not sure about breast pumps.

    Thanks for your advise. This is exactly what I have suggested to her and she is also doing. But she is getting exhausted as have ti take care of two and she had c-section.

    We are also considering breast pump..Since she is in a small town we are planning to send from USA which may take a month

    I guess she is not in the favor of spending much money in pump ( electric pump cost arnd 5k-10k) in india as this might be extra burden on her DH , she is not working and have loan.
    Thanks for suggesting. I guess she can also try nipple shield. Is there any specific brand?

    Thank for suggestion Anu,...this is what she is also doing but because of twin and long BF hours she is getting too much tired and no time for rest

    Yeah..we are also expecting that her kids will learn technique soon.

    Breastfeeding is difficult initially but once settled the nipples become numb and its a pleasure to watch the babies sleep half sucking:p Did you try nipple cream Medela Purelan? Its like paraffin and soothes the nipple. You can also take a paracetamol to lessen the pain. I used hands to pump initially since automatic pump was painful. If you don't keep pumping or feeding though, milk becomes diluted and eventually stops. Thats what happened to me after 6 months since my son was hungry even after feeding so I introduced formula.
